Aim: To undertake a literature review of factors that inhibit decision making/choice for women around birth positions during the second stage of labour.
Methodology: A systematic literature search to identify both quantitative and qualitative articles relevant to the aim of the review was conducted using university library searches and the following databases: CINAHL, the British Nursing Database, MEDLINE and ScienceDirect. The search was limited to the last 10 years to ensure appropriate application to current practice. Findings: Five articles were suitable for inclusion. Power and control remain prominent within the relationship between women and health professionals, influenced by a maternity culture that is dominated by a biomedical philosophy. The midwife's preference for positions, along with lack of knowledge, skills and limited resources, including use of birthing equipment, is reported to also have an impact on the position that a woman will adopt during birth. Conclusion: Power relationships, based on entrenched values of medical dominance and women as passive recipients of care, are barriers to the implementation of true choice. The desire for midwifery-led models of care and the importance of the midwife-woman relationship play crucial roles in the decision-making process. These factors should become a crucial element of care provision to ensure the ideal uptake of valuable and respectful maternity care, with strong emphasis on women's preferences of birth position. |
