People often repeat the cliché that France is 10 years late as compared to the US. Or perhaps this only has to do with cultural difference and bias. Anyway there is no such thing as an American dream for midwives in this paper by Steffie Goodman, who investigated the marginalization of certified nurse-midwives (CNMs) in the US. This marginalization is taking place despite ample evidence demonstrating that a midwifery model delivers high-quality cost-effective care. Currently – as of 2010 – midwives attend only 7.8% of births in the US, compared to 50-75% of births in other developed countries. Given the escalating costs of health care and relatively poor maternal and child health indicators in comparison with other developed countries, these findings are disturbing. This article analyses the American paradox which has kept evolving since the introduction of so-called Obamacare in the early 2010s.
