This article will outline the physiology of third stage of labour, discuss postpartum haemorrhage (PPH) and examine acive and expectant management approaches in the third stage of labour. It will also critique the research studies that have influenced management approaches in the third stage of labour. It will conclude by reporting that there is currently a lack of robust evidence examining whether active or expectant management reduces primary PPH and subsequent blood loss at birth in women at low risk of PPH, in high-income countries.
