This case study describes a novel care plan for a pregnant woman with bipolar disorder, who passionately wished to breastfeed while on lithium therapy. The plan involved pausing lithium 48 hours before birth and recommencing on day one postpartum, expressing and disposing of breast milk for eight weeks, and measuring lithium levels in the breast milk. The plan aimed to facilitate a successful breastfeeding experience while ensuring stability of a pre-existing bipolar disorder diagnosis during pregnancy and the immediate postnatal period. This case study aims to illustrate the complexities involved in advocating for this woman's wishes through the development of an individualised and collaborative care plan.
